CareMeridian’s neurorehabilitation and supported living programs in Arizona are community-based residential programs and provide services for people who have sustained a neurological injury as a result of trauma, stroke and other related medical events.
Our Magnolia House location in Phoenix (5 beds) provides daily living assistance and support services to participants who have been discharged from a hospital or other more intensive rehabilitation facility, but still face physical, psychological and social challenges as they work towards increased independence and community integration.
