CareMeridian offers post acute facilities throughout California (Northern — Bay Area; Central — Fresno and Sacramento; and Southern — Los Angeles, Orange County and San Diego).  These facilities provide care to patients with needs ranging from multiple trauma, fractures, tracheostomy care, wound care, and spinal cord injuries to severe traumatic brain injury.   Each location offers a 4:1 patient-to staff ratio, state-of-the art equipment and individualized care plans.

Six facilities in California also feature CareMeridian’s New Start Program for Spinal Cord Excellence, offering a full continuum of care, rehabilitation and training for independent living to patients that have suffered from catastrophic spinal cord injuries.  These patients face unique mental, physical, cognitive and social challenges as they reintegrate into society.
