Post evaluation, our admission process is straightforward
– we work with the insurance company to get authorization,
and then coordinate between the family, Payor and the specific CareMeridian
facility to make the patient transfer at a convenient and appropriate
time.
Each of our facilities are directed by registered nurses with rehabilitation
experience who oversee daily operations, and confer with physicians
and therapists to assess individual care plans. We also have an
in-house RN case manager dedicated to ensuring that our patients
receive optimal yet cost-effective care.
Like almost everything you find at CareMeridian, our internal case
management role is unique in the industry. We don’t just look
at short-term goals – we are dedicated to treating the whole
patient, to addressing their entire needs, and to taking a personal
interest in the lives we touch long after they leave our care.
