Case Managers / Discharge Planners

Our highly-trained case management staff is in close contact with case managers and discharge planners to ensure that care is being provided according to your and/or the Payor’s specific guidelines.

Each of our facilities is directed by registered nurses who oversee daily operations, and confer with physicians and therapists to assess individual care plans. We also have an in-house RN case manager dedicated to ensuring that our patients receive quality yet cost-effective care.

Like almost everything you find at CareMeridian, our internal case management role is unique in the industry. We don’t just look at short-term goals – we are dedicated to treating the whole patient, to addressing their entire needs, and to taking a personal interest in the lives we touch long after they leave our care.
