CareMeridian is a leading provider for the catastrophically ill or injured patient requiring short and long-term nursing and rehabilitation services. We specialize in rehabilitation and medical management for adult and pediatric patients suffering from:

  • Pulmonary injuries (ventilator and tracheostomy weaning)

CareMeridian is an alternative to continued hospitalization, traditional nursing home/subacute placement or home nursing care. Our unique model of care (each center has no more than 18 beds and maintains a 1:4 staff to patient ratio) facilitates a highly customized treatment plan.

We are able to manage high-cost, medically complex patients who would otherwise need to remain in the acute care setting or be moved to a long-term acute care facility (LTAC) – both options increase cost. In many cases, we are able to accept patients directly from the ICU or Med/Surge Unit. We provide a valuable alternative when you have a difficult discharge. Our network rates average approx. 40 percent below our acute care counterparts.

At CareMeridian we pride ourselves on the individual care and attention we give our patients. One of the most compelling aspects of our program is the continuity of care provided by the same team in a small facility that allows each staff member to work with every patient throughout their stay. Every member of our team knows each and every patient, and they are personally involved in every aspect of their care. CareMeridian does not unnecessarily fragment care by having a stream of caregivers providing treatment in their specialty area only. Our model allows nurses to treat the whole patient.

The individualized treatment program and team approach allow the staff to address all issues as opposed to having segregated disciplines address only their problem area (ex: assessing pain level while adjusting vent settings/evaluating hydration or cognitive status while suctioning, etc). This is more comprehensive and functional for the patient. Our team is specialized in this level of care and has the resources to assist in areas outside of the usual and customary training.
