Case managers or discharge planners direct
most patients to us. And we receive many referrals from those
in the community who know us, and from physicians and families of
patients who know our unique approach firsthand.
Why? Because we are able to achieve a positive clinical outcome
within the shortest length of stay possible. We can get these patients
to an increased level of functioning at a lower cost, and reduce
the number of times they go back to the hospital.
CareMeridian offers up to five levels of care based on medical
complexity and therapy needs. We treat people of all ages who suffer
from catastrophic illness or injury, including:
- Traumatic Brain Injury
- Pulmonary injuries (ventilator and tracheostomy weaning)
- Neuromuscular Disorders
- Spinal Cord Injury
- Congenital Anomalies
We typically handle patients who are difficult to place in conventional
care programs or facilities – often CareMeridian is the only
alternative they have. If the referral looks to be a possible candidate
for our care, we then begin our evaluation
process.
Once a patient is admitted – and sometime ever prior –
our internal case managers go to work, providing regular reports
to the family, Payors and their representatives. Our case managers
have all had experience providing hands-on care, acting more like
“project managers,” watching and making sure everything
is progressing the way it should. They research benefits questions,
monitor therapy, and relay general issues and information between
the caregivers and the family.
Our services are unique, like almost everything we do. But the
goal is always the same – to provide the highest quality care
in a non-institutional environment, producing the best possible
outcome in the most efficient manner.
