Since 1989, CareMeridian has been the innovative leader in post acute neuro-rehabilitation for patients suffering from catastrophic injury or illness. Many of these patients are too fragile or medically complex to meet the criteria to enter an acute rehabilitation program and are difficult to place in traditional post acute settings.

We have pioneered a unique model by blending the aesthetics of a custom built home with the capabilities of caring for high acuity patients. Our focused expertise and high staff to patient ratio enable us to manage high-cost, medically complex patients who would otherwise need to remain in the acute care setting or be moved to a long-term acute care facility (LTAC) – options that both increase cost. In many cases, we are able to accept patients directly from the ICU or Med/Surge Unit.

We work with a variety of Payors, from worker’s comp to commercial insurance, IPAs and others. CareMeridian is a valuable option when you have a difficult discharge – our network rates average approx. 40 percent below our acute care counterparts.
